I agree with you on all accounts! I see the last name thing all the time. It used to be a rule that we'd add an apostrophe to make the plural of some words (like CD's and Smith's), so I can understand why people still do it that way, but now we only use apostrophe to indicate plural when you have singular low case letters (dot your i's and not dot your is) or when you need to pluralize a word where just adding an s would cause you to misread the word ("did you put any do's on your list?" and not "did you put any dos on your list?"). Otherwise, the rest of the time we just add s or es.
